Welcome to discharge facilitation, part of the medicines optimisation training programme. The programme has been designed to facilitate the development of learners to be able to deliver the medicines optimisation service of discharge facilitation in a patient facing/clinical role. It is a holistic, competency-based training programme comprising of skills relating to the safe discharge and transfer of care medications.
The following entry requirements to ensure that learner has sufficient experience and has developed attributes and attitudes to accept responsibility of the role.

Learners will be provided with a learning pathway (LP) on acceptance onto the programme. The learning pathway combines all the training resources and programme templates necessary to complete the training programme. Please contact the training programme director if you have any difficulties or questions regarding the learning pathway.
